Medical report request

Use this service to request a medical report.

Please see our non-NHS work policy. The majority of medical reports are deemed as ‘private’ therefore a charge will be incurred. Once we have received your request, we will let you know how much the requested report will cost. If a charge is incurred, then we will ask for payment in advance.

• are registered at the surgery

We’ll ask you for:

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
