Care Co-ordinator
The Rural West Primary Care Network (PCN) is seeking to recruit a PCN Admin and Data Care Co-ordinator, to work within our PCN’s multidisciplinary healthcare team and enhance our friendly, busy network of practices. The Network consists of two GP Partnerships: Tadley Medical Partnership and Watership Down Health with surgeries in Kingsclere, Overton, Oakley and Tadley, Hampshire.
The main purpose of this role is to support the Rural West PCN Manager and to act as a key point of communication between the Clinical Directors, GPs, the PCN Manager and Practice Business/Managers within Rural West PCN. The post holder will work as an integral part of the PCN’s management teams, working alongside the PCN Manager, the Digital & Transformation Lead (DTL) and the Executive Team to provide administrative support and the
supply of relevant data services across the PCN.
The postholder will be based within both Practices, carrying out a range of administrative tasks in support of the PCN and its Executive Team, including the management of meetings and general day to day work. They will have overall responsibility for arranging the monthly PCN led Executive Team meetings (and other MDT meetings as required); this includes scheduling
the meetings, managing the meeting agenda items and providing minutes of the meetings. Administrative support to the Rural West Network will also be required as part of the ongoing evolvement of Integrated Neighbourhood Team working as this develops, including any required meeting administration. The ability to work confidently, confidentially and effectively
in a varied and sometimes challenging environment is essential.
The postholder will also assist the PCN’s Digital & Transformation Lead (DTL) with the maintenance and development of IT based information systems and have responsibility for key performance data. The post holder will need to have an understanding of and the ability to extract data from various systems, for use in driving improvement and assessing impacts and provide a range of performance/activity data and reports relating to patients with longterm health conditions.
Applicants will need excellent administration, interpersonal and communication skills and be highly organised, energetic and discreet. The postholder will have experience of working in a primary care setting, ideally in health, social care or other support roles. The post is full time (i.e. 37.5 hours per week, Mondays to Fridays) and requires travel within the Network to the surgeries of both Partnerships, so flexibility of working and access to private transport is essential.

GP Locum – Maternity Leave
We are a friendly, supportive training practice in Hampshire and are looking for a GP Locum to cover maternity leave, working up to six sessions per week for approximately four to six months.
We are happy to be flexible around days and working times for the right candidate.

Full/Parttime Practice Nurse (Chronic Disease Management experience – desirable)
We are a friendly and supportive training practice in Hampshire and are looking for a Practice Nurse to join our team.
Experience and skills in chronic disease management would be desirable, but are not essential, as we are happy to support the right candidate to develop their skills and experience within the role.
Start date: As soon as possible
Hours: Part-time / full-time (up to 37.5 hrs) per week
Working pattern: Monday to Friday – flexible working pattern available plus ad hoc extended hours

This is an exciting opportunity to join a fantastic, forward-thinking and diverse primary care team at Watership Down Health.
Due to our expanding patient population and increasing demand across primary care, we are looking for an experienced Practice Nurse with a strong background in long-term conditions, to join our friendly and supportive clinical team.
The role is available part time or up to full-time hours per week and will involve working across our three sites:
- Overton Surgery
- Kingsclere Health Centre
- Oakley Surgery
A full driving licence and access to a car are essential, together with the flexibility to work across our three sites and support the needs of our busy practices.
About the Role
The successful candidate will ideally have previous experience working within Primary Care and will be confident managing patients with long-term conditions.
The role will include:
- Chronic disease management, particularly Asthma, COPD and Diabetes
- Supporting the practice in achieving QOF and other quality targets
- Childhood and adult immunisations
- Cervical cytology
- Wound care and compression therapy
- Infection prevention and control
- Associated clinical administration and patient follow-up
- Working collaboratively with the wider multidisciplinary team
- Providing safe, effective and patient-centred care
Essential Requirements
- Current NMC registration
- Previous experience working as a Practice Nurse in Primary Care
- Experience in chronic disease management
- Good experience in respiratory disease management
- Competence in childhood and adult immunisations
- Competence in cervical cytology
- Experience in wound care and compression therapy
- Good communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Flexible, motivated and committed to providing high-quality patient care
This role will require an Enhanced DBS Clearance.
Full driving licence and access to a car. Flexibility to work across our three practice sites.
Desirable
- Experience of insulin initiation and diabetes management
- Experience in infection prevention and control
- Experience of working within a GP training practice
