Dear Patients,

We are undertaking a piece of work to try and get up to date email addresses and mobile telephone numbers from patients where these are missing from individual patient records. Having these details, mean that we can more effectively correspond with you. We recommend an email address that only the patient uses.

We appreciate that being asked for email and mobile, can cause anxiety so we share with you the message that is going out via our communication tool – Accurx.
